How to Apply for Assistive Devices & Wheelchair Program?

Applying is very easy. It only takes around 5 minutes. Just follow these simple steps below. You can apply online, by visiting an office, or by calling the helpline.

1. Visit your nearest Social Welfare and Bait-ul-Maal office
   You can go there in person and apply directly.

2. Apply online at the official website
   adwc.punjab.gov.pk

3. Call the Helpline
   Dial 1312 and ask how to apply. They will guide you.

If You Are Applying Online, Follow These 5 Steps:

Step 1: Visit the Website

You can use your phone, tablet, or computer.

Step 2: Fill in Your Details

You will see an online form. Fill in:

• Your full name

• Your CNIC number (or B-form, if you’re a child)

• Your mobile number

Make sure all your details are correct.

Step 3: Upload Your Disability Certificate

You will need a Disability Certificate issued by the District Assessment Board.

• Take a clear photo or scan of the certificate

• Upload it in the form

Step 4: Select the Device You Need

You will see a list of assistive devices like:

• Wheelchair

• White cane (for visually impaired individuals)

• Hearing aid

• Other supportive tools

Just select the one that suits your need.

Step 5: Submit the Form

After everything is filled and uploaded:

• Click the Submit button.

• That’s it! You’re done.

Now just wait. You will receive a confirmation message either through SMS or email.
